SUMMARY
The Resource Navigator has the primary responsibility to provide program information and support to the Community Outreach Fund recipients. Such activities include but are not limited to connecting participants to available resources.
RESPONSIBILITIES
-Cross-collaborate with internal and external programs to refer and link community members to need-driven supportive services. Establish connections with community service providers to develop and enhance program resources.
-Complete Community Outreach Fund documentation and assessments according to policy and procedure and document data as appropriate within all required systems.
-Document participant interaction, services provided and external referral outcomes in real time.
-Identify and address barriers to access by offering referrals and advocacy when needed.
-Maintain confidentiality at all times. Transmit applicant/participant information in an appropriate manner with respect for individual privacy and dignity.
EDUCATION, KNOWLEDGE, SKILLS, ABILITIES
Associate or bachelor’s degree. Proven administrative support and quality improvement experience and time management skills to complete a high volume of varied responsibilities in a fast-paced work environment. Additionally, the Housing Program Coordinator will:
-Demonstrate excellent verbal and written communication skills, including the ability to effectively articulate CTL’s mission, vision, and message.
-Demonstrate proficiency in quality improvement, performance measurement, and/or program evaluation and demonstrate advanced computer skills (Windows operating system and Office products, including Word, Excel, and Outlook). Ability to learn and use any other software programs needed in the operations of CTL programs.
-Work independently with minimum supervision and maximum accountability. Possess excellent interpersonal skills and the ability to work well with all levels of management, staff, and families. Prioritize and execute multiple projects under demanding deadlines with attention to detail.
